U.S. President JD Vance announced Wednesday that the Trump administration intends to withhold federal funding for government health insurance programs from states that do not comply with its anti-fraud initiatives. This policy targets states deemed to be insufficiently addressing fraud within their Medicaid and Medicare systems, potentially impacting millions of Americans.

The Centers for Medicare and Medicaid Services (CMS) has already paused new enrollments for hospice and home healthcare agencies for six months to investigate potential fraud. Vance indicated that states failing to demonstrate seriousness in combating fraud could see their Medicaid and Medicare funding reduced. The administration plans to audit Medicaid Fraud Control Units (MFCUs) in all 50 states to assess their effectiveness in pursuing known abuse of Medicaid funds.

This action follows a previous withholding of over $300 million in funding from Minnesota due to non-compliance with other requirements, though that hold was subsequently stayed by CMS. Experts note that while scrutiny of state-level fraud investigations is not new, this approach emphasizes financial penalties for states. The Department of Health and Human Services (HHS) has not specified the criteria for non-compliance or the full impact on care access.
