Teen Dies in Youth Detention Custody

A 16-year-old boy has died at Feltham Young Offender Institution (YOI) in southwest London after being found unresponsive on Monday night, according to officials. Paramedics treated him at the institution before he was taken to hospital. He died shortly before midnight.

Police and the prisons ombudsman have opened investigations into the circumstances of the death. The Metropolitan Police said the death is being treated as unexpected and that inquiries are continuing as authorities pursue all lines of investigation to establish what happened.

A post-mortem examination has been scheduled for Friday, April 24. Officials have not released further details about the teenager or the events leading up to him being found unresponsive.

The death is significant in the context of youth custody in England and Wales. Officials said it is the first death of a child in a YOI in England or Wales since June 2019.

Charities including Inquest and Nacro pointed to broader concerns about the safety of children in state care and in youth custodial settings. They said the case underlines the need to reassess how children are managed within the youth justice system.

Inquest and Nacro called for a move away from punitive approaches and toward community-based support services. They also urged reforms that would reduce reliance on larger institutions such as Feltham, arguing instead for smaller, secure children’s homes.

For now, the immediate focus remains on the ongoing investigations by police and the prisons ombudsman, and on the findings expected from the post-mortem examination. Until those processes are completed, key questions about the circumstances of the teenager’s death remain unresolved.
